Seos Contact Form is easy to use with all the basic field – Name, Phone Number, Email, Website, Subject, Message, Select Options, Radio Buttons, Checkbox, Enter The Sum.
Seos Contact Form allows you to select the required fields, Custom text – Contact Form Title, Custom text – Required,
Custom text – Your message is send, Custom text – Your message is not send, Admin Email, Shortcode – Include form in your website.

Insert [seos_form] on a page to display a form to add citations.

安裝
Install via Plugins > Install New
- Search for “Seos Form”
- Click the “Install Now” link
- Click “Activate Plugin”
Via ZIP / FTP
- Unzip the ZIP file and drop the folder straight into your wp-content/plugins directory.
- Activate the plugin through the ‘Plugins’ menu in WordPress.

The plugin is simple and seems that the free version doesn't have ways to manage the styles using the backend, but I've managed to change the styles trough custom css so it's OK.
Had a question and the author replied in less than 5 minutes. May be coincidence but I was impressed
